Submit or update a COVID-19 immunization record

Who should use this form

You will be required to upload an image of your official vaccination record with the details of your previous dose(s). If you received your immunization in Canada but outside BC, ensure you submit your proof with a QR code for faster processing.

You may be contacted by our call centre if any of your information is missing or needs further verification before your record is processed.

Your immunization record will be reviewed and updated in the Provincial Immunization Registry. Processing times may vary from 10-14 days.

Once your record is updated, if you meet the eligibility criteria (see below), you will be able to access your immunization record online through Health Gateway and use the BC Vaccine Card.



If you were vaccinated outside B.C.

You must complete this form before you can get a BC Vaccine Card. You can continue to use your out-of-province proof of vaccinationin the meantime. Even if you have been vaccinated against COVID-19 outside Canada, you may still need to receive vaccinations here.

BC is following the World Health Organization’s (WHO) approved COVID-19 vaccines list to decide whether or not it is recommended that you receive additional COVID-19 vaccination. Some Sinopharm products have not yet been approved by the World Health Organization (WHO). In BC, Pfizer/BionTech/Comirnaty is considered the same product; please select the trade name that most closely aligns with your record.

You need a B.C. Personal Health Number (PHN) to complete this form. If you do not have a PHN, call 1-833-838-2323 and one will be provided to you.

If your proof of vaccination is not in English or French

All submissions must be translated to English or French. If your proof of vaccination is in another language, do not upload it.

You must submit a certified translation in English or French for eligibility for a BC Vaccine Card to be considered. Please include:

  • The original proof of vaccination
  • The certified translation, including the stamp or membership number of a professional translation association

You must confirm that the information provided is accurate

You will be asked to confirm that the information you are submitting is accurate. Providing false information may make you subject to prosecution under the Criminal Code of Canada.

Your personal information is being collected under sections 26 (c), (e) of the Freedom of Information and Protection of Privacy Act, for the purposes of registering and scheduling your COVID-19 vaccination.
