Use this service to add or update a vaccine record for the B.C. Provincial Immunization Registry (PIR). Your vaccination records will be reviewed by a health care professional, who will update your provincial health records.

What information will you need?
You must provide accurate information. You will be asked to confirm that the information you are submitting is accurate. Providing false information may make you subject to prosecution under the Criminal Code of Canada.
- Your B.C. Personal Health Number. If you don’t have a PHN, call 1-833-838-2323
- An image of your official vaccination record (for example, an immunization card or record from a physician’s office), showing the details of the dose(s) you received. It must be in English or French or a certified translation.
- Personal and contact information
Your personal information is being collected under sections 26(c) of the Freedom of Information and Protection of Privacy Act, for the purpose of registering and scheduling your vaccination. Personal information may be shared with personnel providing support services and follow-up. For a privacy-related matter only, please contact a privacy officer at 1-855-229-9000.
